Signatures and attestation
Drawn e-signatures with name and time, and an attestation statement the person must accept before submitting.
Signature questions
Add a Signature (drawn + name) question to any checklist, for example Auditor, Surgeon or Nurse handing over. See Question types.
When filling in the checklist:
- Type your Full name.
- Sign with your finger or mouse in the dotted box. Tap Clear to start again.
Both the name and the drawing are needed to answer a required signature.
The signature is saved as an image with the time it was signed. It shows on the inspection report as the drawing, the name and Signed with the date and time, and is printed on the PDF.
Note: Signatures are never copied by Copy answers from last time. Each one must be signed fresh. See Run an inspection.
Attestation statement
An attestation is a statement the person must accept before they can submit, for example “I confirm I personally checked each item at the point of care and these answers are true and complete.”
Set it up
- Open the checklist in More → Manage → Checklists.
- Type the statement in Attestation statement (optional).
- Save. Leave it empty for no attestation.
When filling in
The statement appears above Submit inspection with a tick box and the line Signed electronically as followed by your name. You must tick it to submit.
On submit, CareList stamps your name, the date and time (to the second) and the exact statement you accepted. The report and PDF show Attestation with Electronically attested by your name and the time. Changing the statement later does not alter past inspections.
Ready-made examples
In the demo hospital, these checklists already carry an attestation:
- WHO Surgical Safety Checklist
- Medication Administration Check
- Discharge Readiness Checklist
- Pre-surgery Checklist (NABH)
In the demo diagnostic centre:
- Report Verification & Release
- Critical Value Communication
All NABH audits end with a drawn signature, usually the Auditor's.
Tip: Combine an attestation with Requires approval so a second person signs off. See Approvals.
