Build a checklist

Create a template, add questions, and publish it.

Managers build checklists under More → Manage → Checklists.

Create and edit

  1. Choose to create a new checklist and give it a name, optional description and category.
  2. Tap Add a question and pick a question type.
  3. Type the question text. Add help text if people need guidance.
  4. Mark it required if it must be answered.
  5. Reorder questions with the move controls, or remove ones you do not need.
  6. Tap Save.

Publish

A checklist can only be assigned once it is Published. Switch Published on and save. Switch it off to stop new assignments without deleting anything.

Other settings

  • Scoring and pass threshold: see Scoring and pass marks.
  • Requires approval: completed inspections wait for a manager. See Approvals.
  • Conditional questions: see Show a question only when needed.
  • Layout when filling in: All questions on one page, or Wizard – one question per screen with a progress bar, Next and Back. The wizard suits long bedside checklists such as the WHO Surgical Safety Checklist or Vitals Chart. See Run an inspection.
  • Attestation statement (optional): a statement the person must tick before submitting, stamped with their name and the time. See Signatures and attestation.

People filling in a checklist can also Copy answers from last time for the same patient, order, equipment or location, and annotate photos. Both are always available; there is nothing to switch on.

Copy, export and delete

From the checklists list you can duplicate a checklist (the copy starts unpublished with its own QR code), export it, or delete it.

Tip: Rather than starting from zero, install ready-made checklists from the template library.
